New Patients

Welcome to Senior Adults Specialty Healthcare! As a new patient there are some important forms that we need you to complete before your first visit.

You can complete these online and submit them electronically or you can print them out and either fax, mail, or hand deliver them but they must be to our office 48 hours before your scheduled appointment or your appointment may be rescheduled. 

All forms must be completed in its entirety but please  choose  either the psychiatric history or the neurological history depending on your appointment.

Established Patients

If you are an established patient and need to talk to your provider about your care, please call the office and ask for your provider and if you leave a message be sure to include the reason your are calling. The provider’s  medical assistant will call you back to get more information to provide you with the best response possible from your provider.

OR you can reach your provider directly by going on the Practice Fusion portal and emailing them directly. This is the preferred way to contact your provider.

If you need a refill on a prescription – please contact your pharmacy to initiate a request. If new refills are needed they will contact our office electronically.
